Job Description::
This position is responsible for the development, implementation, and facilitation of an Individual-Centered Service Plan (ISP) process for persons served, ensuring integration of service planning through the involvement of the individual, family, guardian, other service providers, agency staff, and others identified by the individual. This position is based primarily at the CILA homes and requires a flexible schedule (i.e., days, evenings, and weekends). It requires 24/7 on-call cell phone responsibility for medical, behavioral, and clinical emergencies regarding residential consumers. Provides leadership for CILA House Supervisor and DSPs in the development and implementation of individual and group programming for the individuals served.POSITION RESPONSIBILITIES:•Ensure the delivery and/or coordination of all CILA services in compliance with DHS & 116, CARF standards, agency mission, agency policy & procedure, program guidelines, and best practice.•Responsible for the development, implementation, and facilitation of Service Plans for assigned individuals, ensuring integration of habilitation planning and provision of services through person centered planning. Monitor and modify plans in such a manner as to ensure that services outlined continue to meet individuals’ needs and reflect the priorities of the individual served. Responsibilities as Residential Service Coordinator/QMRP include:°Complete the Pre-Meeting Personal Preference Interview as needed prior to the scheduled Service Plan meeting in order to determine strengths, abilities, needs, interests, preference, and choice in service provision.°Ensure that Service Plans are developed with the input of the person receiving services or a significant other identified by the individual who can best represent their ideas, concerns, abilities, preferences, cultural background, and desired outcomes of services.°Schedule and convene the Service Plan meeting in a timely manner and in accordance with regulations as set forth by the funding source, licensing entity, and/or accreditation standards.°Secure input, reports, and information from all persons involved in service delivery prior to Service Plan meeting.° Ensure that the Service Plan meeting is a coordinated effort involving the individual and others involved with service implementation and ongoing supports. ° Maintain regular communication with all persons who may be involved with the individual and those requested by the individual or their guardian to participate in the development and/or implementation of the plan. ° Assume responsibility for implementation of the plan.° Assist the individual to become adequately orientated to their Service Plan.° Enable the person’s plan to proceed in an orderly, purposeful, and goal-directed manner. ° Review the plan’s responsiveness to the needs and preferences of the person with their input on goal status based upon performance and interest, and satisfaction with services as indicated through the Quality Assurance Survey process. ° Schedule and convene team meetings in response to identified needs or modifications to the Service Plan. ° Identify methods/techniques to be utilized for plan implementation. ° Facilitate any program modifications including internal program transfers, exits/discharges from agency services, or service enhancements. Ensure that information is communicated in a manner which promotes a smooth transition. ° Maintain confidentiality with respect to the individual, their guardian, and the record of the person served. ° Consider safeguards to ensure that the individual’s program and interactions are free from abuse and neglect. ° Advocate on behalf of the individual to secure requested or needed services. ° Ensure that services promote and protect the rights of the individual served. ° Prepare monthly information packet for psychiatrist; facilitate psychiatric meeting regarding all individuals on caseload.• Participate in the Intake process for screening, program placement, and appropriate movement of individuals served. • Provide staff with direction, training, and support needed for ISP implementation, data collection, and provide follow-up on ISP revision. Work cooperatively with staff to ensure appropriate access to services as identified during the ISP process. • Function as advocate ensuring protection of an individual’s rights while providing a training element to empower persons with disabilities to understand and protect those rights. • Communicate in a professional, respectful, and effective manner promoting an atmosphere that is responsive to the needs and desires of individuals served. Ensure that all communication, oral and written, not only complies with state, agency, and departmental requirements, but maintains the confidentiality of those receiving services. Maintain all required or necessary documentation per state, agency, and departmental requirements. Encourage, facilitate, and support the development or maintaining of natural supports.• Make decisions that reflect compliance with all regulatory, agency, and departmental requirements. Respond in a timely, effective, and proactive manner to address all issues or concerns presented by individuals served, families/guardians, staff, other service providers, and other agency personnel. Ensure that financial decisions/practices reflect budgetary constraints. Respond and take immediate action to address all issues that jeopardize the safety, well-being, and dignity of individuals served. • Complete monthly filing and monthly monitoring reports in a timely manner. • Assist in maintaining individual program books, medical appointment book, and other CILA home files.• Ensure individuals receive annual physical, dental, and follow-up appointments in a timely manner and in accordance with Rule 115. Accompany individuals to specialty medical appointments and other appointments as needed. Communicate any needed medical information to RN, guardian, or other family members. • Develop, implement, and monitor behavioral intervention plans for individuals. • Monthly prepare information packet for psychiatrist; facilitate psychiatric meeting regarding all individuals on caseload. • Facilitate monthly Community Support Team meetings for assigned CILA homes. Ensure bi-weekly visits to ICILA consumers’ homes.• Remain aware of current trends, developments, and best practices within the field, using this information to enhance the quality of services provided in the CILA program. Monitor all aspects of the CILA Program to ensure compliance with regulatory and agency standards recommending and following through on recommendations for correction of any deficits. • Provide input into the development of departmental guidelines, outcomes, and practices. • Comply with all Sertoma policies and procedures as established.

Qualifications::
•Bachelor’s Degree in Human Services field of study required and must meet all QMRP requirements.•Minimum of one year experience working with individuals with developmental disabilities and/or mental illness.•Supervisory experience preferred.•Effective communication skills, both written and oral.•Demonstrate ability to manage multiple priorities.•Prior experience in a residential setting, preferably in a supervisory role.•Valid driver’s license with a good driving record and insurance documentation. •Proficient in the use of computers, software applications, and working knowledge of Microsoft Word and Outlook, with good typing skills.
